• Para acceder inmediatamente a su historial médico, visite Mi Salud en línea o nuestra aplicación Healow.

• To request your records to be emailed to you or another provider, fill out our Electronic Records Request form.

• Download and complete the Health Information Release form in English or Spanish, fax or mail it to our address listed on the form, or bring it to one of our clinics.
   

Immunization Records:

• You can request your complete immunization records directly from the state immunization public portal.

• Una vez que haya presentado su solicitud, podemos tardar hasta 30 días en devolverle los registros.

• Si solicita imágenes y tiene acceso a PowerShare, realice su solicitud a través de PowerShare.

• Salud requests Continuity of Care records requests to be submitted in writing. Please download and complete the Continuity of Care form  and fax it to 303-227-6426.
